2006 Mortgage Lender Annual Report 

Instructions 

All portions of this form must be complete when forwarded to the Division of Banking.  Incomplete forms will not be accepted for processing and administrative penalties will be assessed until the report is complete.   

It is the licensee’s responsibility to promptly notify the Division of Banking of any change in contact personnel for the following areas:  Licensing, Compliance, Consumer Complaints, Records Custodian and Examination.  Updates to mailing address, telephone, fax and email are also required for each of the referenced individuals. 

Report forms are available on the Division of Banking’s website and are not mailed to licensees.   

An administrative penalty of $100 for late filing of this report will be assessed for each day after March 15, 2007.

Provide a listing of all mortgage loans originated or funded in West Virginia for the period July 1, 2006 through December 31, 2006.  The listing should include:  Date Loan Closed, Borrower’s Name and Address, Original Loan Amount, Broker’s Name and Appraiser’s Name if the appraiser’s main office is located more than 75 miles from the subject property.  (Note:  The loans provided in response to this question should not be the same as provided with the 2006 WV Mortgage Broker Annual Report if your organization holds licenses as both a lender and a broker.)

Provide a listing of all West Virginia mortgage loans purchased or accepted for the period July 1, 2006 through December 31, 2006.  The listing should include:  Date Loan Closed, Originating Lender/Broker, Borrower’s Name and Address, and Original Loan Amount.  (These loans are not included in the fee calculation.)
